Sites can have this icon for a variety of reasons. It means that the site is not what you necessarily expect. An example is a site with some free galleries, but which is in effect merely a means to draw traffic to a paysite. Because of the free galleries, the site still has value (and is thus listed), but you are advised to exercise caution when navigating it, in order to avoid landing somewhere you didn't set out to.
